Who is lower blepharoplasty suitable for?

The best candidates are in good general health, do not smoke, have realistic expectations about what surgery can achieve, and are troubled by one or more of the following:

  • Persistent under-eye bags or puffiness
  • Excess or loose lower eyelid skin causing a crepey or wrinkled appearance
  • Fine lines and skin laxity on the lower eyelid
  • A generally aged or fatigued appearance around the eyes

Lower blepharoplasty is not suitable for everyone, and a thorough consultation and examination is essential before surgery is agreed. Patients with certain eye conditions — including significant dry eye, previous eye surgery, or conditions affecting the eyelid position — require particularly careful assessment.

A note on non-surgical alternatives

For some patients non-surgical options may  provide meaningful results without surgery.

These include

  • Tear trough filler for under eye hollowing
  • Resurfacing laser for wrinkles and crepey skin
  • Skin tightening treatments such as Secret Pro

Ms Brady will discuss all relevant options with you honestly at your consultation.

YOUR PROCEDURE

What to expect on the day

Lower blepharoplasty is performed as a day case under local anaesthetic and sedation or general anaesthetic. You should arrange for someone to drive you home. During the surgery Ms Brady will carefully removes or redistributes fat pockets causing puffiness, addresses any excess skin, and tightens the lower lid where needed. Fine sutures close any external incision. Both eyes are treated at the same visit. The procedure typically takes 2-3 hours in total.

RECOVERY

Your recovery timeline

Recovery from lower blepharoplasty is very individual — some patients sail through with minimal bruising while others experience more significant swelling. Both are entirely normal and do not reflect the quality of your surgery or your final result. What matters is that you give your body the time and care it needs to heal properly.

Be patient with the process

The most common reason patients feel anxious in the recovery period is that they compare their swollen, bruised early appearance to their hoped-for final result and feel disappointed. This is completely understandable — but almost universally unfounded. The transformation from day three to month three is remarkable. Trust the process and resist the temptation to judge your result before the swelling has fully resolved.

DAYS 1-3

Swelling and bruising are at their most significant. The eyes appear puffy, sometimes quite dramatically so — this is normal and temporary

DAYS 4-7

Swelling begins to reduce noticeably and bruising changes colour as it fades — yellow and green tones replace the initial dark purple. Sutures are removed at around day 5–7 — this is quick and comfortable.

WEEK 2

Bruising fades considerably and residual swelling continues to settle.

WEEKS 3-4

Residual swelling is subtle and mainly noticeable only to the patient. Scars begin to fade from pink to white. Most patients have returned fully to normal life.

3-6 MONTHS

Scars continue to mature, soften, and become less visible. The final result is fully apparent — lower lids look smooth, rested, and naturally youthful. Swelling has fully resolved. Most patients find the result at six months exceeds their expectations at six weeks.

Realistic expectations

The best outcomes in blepharoplasty come when patients and surgeons share realistic, well-aligned expectations. The goal is always to enhance and refresh — not to change your fundamental appearance, erase all signs of ageing, or produce a result that looks different from you. A thorough consultation, good communication, and an honest discussion of what is achievable are the foundation of a successful outcome.
